If you are new to The Healthy Bone and interested in learning more about our services, please complete both the K9 Adventure Client Information form and the Pet Information Disclosure (one P.I.D. form per pet please).
Before scheduling your first hike with The Healthy Bone, and in addition to completing the aforementioned Client Information and Pet Information Disclosure, please be sure to complete the K9 Adventure Program: Questionnaire and Consent Form, The Veterinary Release Form, the Program Policies/Legal Considerations form. You must also read through and sign The Healthy Bone: Waver and Release of Liability form Thank you!
