Please Note:

For the purposes of this document, the terms Client, Owner, Pet Owner, and Customer are synonymous with the person contracting services for one or more domestic animals. Handler, Sitter, Employee, Driver, and Attendant are synonymous with the individuals providing Pet Care Services on behalf of The Healthy Bone.

Terms of Document Coverage *

The terms of this document apply to all the pets owned by the client, including any and all new pets that the customer obtains on or after the date this document was signed, at any and all locations the owner designates for service.

Client Forms *

The following forms must be completed in full before any services are provided:

Client Information Form (one per family)

Pet Information Disclosure (one per pet)

Veterinary Release (one per pet)

Questionnaire and Consent (one per family)

Program Policies and Legal Considerations

Program Waiver and Release of Liability

I understand that no services will be provided until I have completed all required Client Forms.

Unpaid Services *

Services may be disrupted if payment has not been received within a reasonable amount of time. Please do not hesitate to contact us should you require special arrangements be made.

Cancellations *

Cancellation Charge Schedule (% applies to the service period TOTAL)

0-48 hours prior to any scheduled Holiday Service: Payment in full will be charged (No refunds if paid in advance or charged the balance due in full)

0-24 hours before day of regularly scheduled services: Payment in full will be charged (No refunds if paid in advance or charged the balance due in full)

Early Return *

Reservations are made to plan for space and handler availability, therefore, clients returning home early will be required to pay for the remaining reserved amount of time.

Method of Payment: Credit Cards *

If paying for services rendered by Credit Card, I hereby grant The Healthy Bone permission to charge the credit card I have on file with The Healthy Bone for all monthly services rendered. I understand that there is a 3 percent charge for each credit card transaction, which will be added to my monthly bill.

Method of Payment: Checks *

The Healthy Bone will impose a charge of $35, along with any additional fees incurred, due to a Returned Check.

Veterinary Services *

The Healthy Bone is authorized to seek any emergency veterinarian assistance needed during visits, at the cost of the client, from any veterinarian as chosen by the sitter. Please know that The Healthy Bone will do everything within its power to utilize your designated veterinarian clinic, as defined within the Veterinary Release form. Should we be unable to schedule an appointment in a timely fashion or should your pet(s) require immediate care, The Healthy Bone reserves the right to visit the nearest veterinarian location.

Injury by Pet *

Pet Owner will be responsible for all medical expenses and damages resulting from an injury to a Pet Sitter, or other persons, by the Pet. Customer agrees to indemnify, hold harmless, and defend The Healthy Bone, in the event of a claim by any person injured by the Pet.

Dangerous Pet/Situation *

The Healthy Bone reserves the right to terminate this contract at any time if the Pet Handler, in his/her sole discretion determines that Owner’s pet poses a danger to the health or safety of itself, other pets, other people, or the Pet Handler. If concerns prohibit the Sitter from caring for the pet, the Owner authorizes the pet to be placed in a kennel (or previously arranged locale), with all charges (including but not limited to transportation, kenneling, tranquilizing, treating, accessing, and liability) to be the responsibility of the Owner.

Future Services *

I authorize this contract to be valid approval for services so as to permit The Healthy Bone to accept all future telephone, online, mail or email reservations and enter my home without additional signed contracts or written authorizations.

Concerns Upon Completion of Service *

Client agrees to notify The Healthy Bone of any concerns within 24 hours of client returning home with pet.

Term and Conditions *

The owner/client states that he/she has read this agreement in its entirety and fully understands and accepts its terms and conditions.

Legal Consideration Agreement and Future Policy Changes *

This agreement is valid from the date signed, and replaces any prior Legal Considerations agreements. Client agrees to any future term changes relayed verbally to the client, mailed or emailed in writing to the client, or posted on The Healthy Bone's website.
